The number of persons infected with hepatitis C virus (HCV) in the United States is estimated to be over 100,000 persons per year, with more than half of the new infections occurring in injection drug users. Transfusion and dialysis are also important methods of HCV spread, and these forms of transfer are associated with an increased risk of progression to chronicity. The “goal” of therapy is to achieve a sustained virologic response, typically defined as undetectable serum HCV RNA 6 months after the end of treatment. A wide range of combination therapies are available, many of which can be extremely effective. To date, the only predictors of response to the combination therapies is baseline HCV RNA levels and/or genotype, and, in most instances, the duration of treatment. Preliminary data from the current year indicate that the two most potent and active anti- HCV agents of the future, peginterferon and ribavirin, are also the most cost-effective for treating HCV. Currently, the allocation of anti-HCV therapies in the US is determined by cost, with the notable exception of the allocation of the current standard therapy, peginterferon and ribavirin, to persons with genotype 1 infection. The cost of therapy as well as the cost of HCV infection per se remains very expensive in the US. During the previous year, we published data indicating that a hypothetical two-year course of peginterferon and ribavirin, the current standard of care for genotype 1 patients, results in an annual cost of US$46,000 in the US, with a lifetime cost of treatment of US$721,000 if therapy is successful. On the other hand, preliminary data from the current year indicate that oral alfa-interferon therapy is cost-effective, with an incremental cost-effectiveness ratio of under $5000 per year of life saved for a 30% response. The overall goal of this proposal is to evaluate the incremental cost-effectiveness of alternative strategies for the treatment of HCV. Patients will be identified through various sources, including drug screens. Patients will be recruited for treatment if they are infected with a genotype 1 virus and have a high risk for HCV-related disease. The three alternative strategies will involve the use of alfa-interferon with or without ribavirin for 12 months, followed by alfa-interferon and ribavirin for 12 additional months. Recognition of tumor-associated antigens (TAAs) by cytotoxic T lymphocytes (CTLs) is a critical step in the establishment of effective tumor-specific immune responses. Current understanding of CTL-mediated tumor rejection implies that the most immunogenic TAAs are tumor-specific or abundantly expressed on tumor cells. Therefore, the identification of CTL epitopes from novel TAAs is an important research focus for the development of cancer vaccines and a critical step for developing personalized immunotherapies.
A CTL epitope is a short sequence of amino acids, typically 8-20, that is presented on the cell surface of an infected or cancer cell as part of a larger protein to a CTL. The peptide sequence of a CTL epitope must be bound and presented in a MHC- I complex to CTL for CTL-mediated immune rejection of tumor cells.
Tumor-specific antigens (TSA) have been identified in most tumor types. This includes tumor-associated antigens (TAAs) on MHC class I, class II, or both. The best known MHC class I TSA are the 9-15, 5-7, and 10-17 sequences (Martin et al., 1991, Srivastava et al., 1986, and Boon and van der Bruggen, 1992).
TAAs on MHC class II are known in fewer cases, and are frequently derived from self-antigens (see Introduction, Chapter 6, for a brief discussion of class II-restricted TAA antigens).
